Physical Health and Medication Management Team (PHMM) – Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust (CNTW)

The Physical Health and Medication Management Team (PHMM) covers Gateshead, East and West Newcastle. The team provide long term condition management and a specific bridging service for people over 18 and under 65 years who have been discharged to their GP.

The service provides a range of mental health treatments to ensure that you receive the treatment that is appropriate for you. Services include: Clozapine clinic, depot injection clinics and nurse led clinic.

Referrals are made by the Community Treatment team or consultant psychiatrist.
