Ways to Wellness
Ways to Wellness runs a number of social prescribing services for people in Newcastle as part of its aim to pilot new healthcare interventions that empower them to take control of their wellbeing. We support:
- People registered at specific GP practices with specific long term health conditions, namely chronic obstructive pulmonary disease (COPD), asthma, Type 1 or Type 2 diabetes or pre-diabetes, heart disease, epilepsy, thinning of the bones (osteoporosis), high blood pressure, and osteoarthritis of the hip or knee.
- People registered at either Cruddas Park Surgery, Holmside Medical Group, Throckley Primary Care centre, or West Road Medical Centre, with persistent physical symptoms such as chronic pain, whose causes are no longer being investigated.
- Inpatients and day patients at the Great North Children’s Hospital up to 15 years of age, with a long term, complex medical condition. We also support these patients’ families and siblings.
A link worker supports you to create an action plan to become healthier and improve your quality of life, including, where relevant, helping you manage your condition. They may work with you to:
- Get involved in local groups and activities
- Access specialist services and support
- Eat more healthily and get more active
- Get support around benefits and welfare rights
