Adult Social Care Transport

The Adult Social Care Transport Service is here to help adults who cannot travel safely or independently using public or community transport.  They make sure that people can get to the care, support, activities and community services set out in their Care and Support Plan, helping people to stay independent, connected and well.

They provide transport for adults who have been assessed as needing support with travel.  Including people who:

  • Have learning disabilities with complex needs
  • Are autistic
  • Have a dementia condition and or significant memory loss
  • Have significant care and support needs in daily life
  • Have physical disabilities and need extra help to travel safely

They can offer assistance with transport to help people access support and activities, including:

  • Travel between home and day services, day centres, resource centres or in-house support
  • Hospital-related travel where Adult Social Care is responsible
  • Transport to education, training or meaningful activities
  • Other travel required to help achieve goals in a Care and Support Plan
