Dementia Connections

Dementia Connections is a service that provides support from Admiral Nurses, who are specially trained to help people living with dementia and their families.

They can:

give practical advice and emotional support
help people who have just got a dementia diagnosis to supporting carers after someone has died .
help you understand dementia and how to cope with its challenges.
be your main contact, making sure support is linked across health and social care services.
tell you about other useful services and speak to professionals for you.
support you to plan ahead and make decisions such as moving into care.
