Community Health and Social Care Direct (CHSCD) give information, signposting and support to adults in Newcastle who are struggling at home with day to day activities. This could be anything from moving around your home, activities of daily living or caring for someone. Call CHSCD for a chat to find out if you or your carer need support. They can signpost you to services that can help. They can help to arrange:
- an assessment to find out what support is available to you (needs assessment)
- help to live independently in your own home (home care)
- short term rehabilitation services (for example, physiotherapy)
- home adaptations, equipment or aids to make life easier
- crisis support
